What forms of payment do you accept?
What is your cancellation policy?
Changes to scheduled follow up appointments must be made at least 24 hours in advance. Failure to do so will result in a late cancellation fee of $100.
I have Medicare and would like to be a patient. Can I see you if I want to pay privately?
Will you complete my disability paperwork?
Do I still need to have a primary care physician?
Are labs or supplements included in the Catalyst Health SPARK Your Vitality Program?
Because lab testing is often partially covered by insurance and both lab testing and supplement plans are highly individualized and vary greatly from patient to patient, these costs are not included in the SPARK Your Vitality Program.
Do you provide urgent or after hours care?

Do you accept insurance?
We are an out-of-network provider and do not accept insurance. We can provide a “superbill” or detailed billing summary upon request, that you may submit to your insurer for partial reimbursement. We cannot assist you with claim resolution for our services.
Some lab tests may be covered or partially covered by your health insurance plan per that plan’s rules, others may not covered.
About Lab Tests
- The specific tests ordered will be personalized to you based on your initial evaluation with Dr. McCarthy and may include blood, saliva, urine or stool testing.
- Lab costs will be reviewed with you and you will have the opportunity to choose which lab tests you feel comfortable paying for.
- Occasionally lab results may indicate a need to follow up with your primary care doctor.
